$120 per 50-minute session
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session? Is approval/referral required from my primary care physician?
Payment
Cash, Venmo, Zelle and all major credit cards accepted for payment. All private pay fees, co-payment/coinsurance payments and applicable deductible amounts will be due at the time of your appointment.
Please contact your insurance company prior to making an appointment to determine if your specific health care insurance policy is in network or out of network for my services. Couples therapy is not covered by health insurance and would therefore require out of pocket payment of $120 per 50 minute session.
Cancellation Policy:
If you do not show for your scheduled therapy appointment, and/or you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
Other Fees:
If I am required to appear in court on your behalf, the fee is $300 prior to the court appearance in addition to the standard rate of $120 per hour. This includes time spent to commute to and from the courthouse, time waiting for the court session to begin, as well as time spent in the court session. If you request FMLA paperwork to be filled out on your behalf, the fee will be $25 per incident. If you or another entity is requesting a copy your medical record, (with your consent) the fee will be $25.00. Phone calls in between sessions over 10 minutes will be charged $120 per hour.
