$90 per 50-minute session

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session? Is approval/referral required from my primary care physician?

Payment
Cash, check and all major credit cards accepted for payment. All private pay fees, co-payment/coinsurance payments and applicable deductible amounts will be due at the time of your appointment.

Currently Accepting: Private payments (ie if you choose not to utilize your health insurance for payment of services) Blue Cross Blue Shield of Texas -Tricare/Humana Military, United Health Care, United Behavioral Health, UMR, Magellan. Please contact your insurance company prior to making an appointment to determine if your specific health care insurance policy is in network or out of network for our services. Couples therapy is usually not covered by health insurance and would therefore require out of pocket payment of $90 per 50 minute session.

Cancellation Policy:
If you do not show up for your scheduled therapy appointment, and/or you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.

Other Fees:

If I am required to appear in court on your behalf, the standard rate of $90 per hour will apply. This includes time spent to commute to and from the courthouse, time waiting for the court session to begin, as well as time spent in the court session. If you request FMLA paperwork to be filled out on your behalf, the fee will be $25 per incident. If you or another entity is requesting a copy your medical record, (with your consent) the fee will be $25.00. Phone calls in between sessions will be subject to a $1 per minute fee.
